Symptoms of PTSD and how they affect the quality of life

The long road Back-my journey through the birth Trauma

My story illustrates the impact that birth trauma can have upon women. On Tuesday, 16th September 1986, two weeks past his due date, my son Jeffrey was delivered via caesarean section, weighing 9 lbs. 13 oz. I had begun pre-natal care at 22 weeks with a doctor who was recommended by a friend. The doctor indicated that he supported my birth plan for a natural birth with a minimum of interventions. My doctor had privileges at two hospitals. During the third trimester, my partner and I toured both. I selected the hospital whose standard procedures were less invasive; no IVs or enemas and they encouraged walking through labour.


A little after midnight on September 16th, my water broke. I had no contractions at this point, but called my doctor. He said that he had a patient in pre-term labour at 26 weeks, who required his attention and that I would need to come to the other hospital instead. After a quick shower we drove there, where my partner dealt with the paperwork while I underwent their standard procedures including an enema and insertion of an IV needle.


By this time, I was having strong, regular contractions. I was then taken to a labour room and placed on foetal monitoring and was unable to walk about. Despite these interventions, I managed the pain and laboured naturally for nine hours. My doctor checked on my progress several times, but was focused primarily upon his other patient. I progressed well and by 7.00, I was 9cm.


At 9.15 when my doctor checked again, I was still 9 cm and the baby had not descended. He said that he could allow me to labour longer but it would make no difference; the baby was too big. After nine hours of intense contractions, a partner that spent more time in the cafeteria or asleep and lying flat on my back staring at the walls, it was then that I lost it and began to cry. Within 15 minutes I was in the theatre and they were administering a spinal. At this point I was so exhausted that once I no longer had the pain of the contractions, I fell asleep.


I remember very little until that afternoon. My vague memories include Jeffrey's first cry and my ex bringing me a picture of him in the recovery room. Because he had swallowed myconium, he had to be observed in the nursery and I did not get to hold him or attempt nursing until that evening. I was not able to have rooming in because of the caesarean and despite the sign in his bassinet that instructed the nurses he was to be breastfed on demand, he was given formula and not brought to me for hours at a time.


At that time, I was greatly disappointed that I had a caesarean and felt as if I had failed somehow as a woman. I was also intensely angry at my doctor. I felt that despite a good relationship during my pregnancy he had let me down when I needed him most. I was especially angry about the tone he had used with me when suggesting that a caesarean was necessary. I felt that the tone of voice he used when telling me that he could allow me to labour longer but it would do no good was bullying. I remember later comparing that experience to emotional rape.


Because my son was in the nursery most of the time and my partner was at work, I was left alone in my hospital room for most of the day. I cried often during my five day hospital stay. I was angry too that the nurses had more time with my new baby than I did. I got to see him for only part of the day. I never got to keep him with me at night. Our breastfeeding experience got off to a rough start because he was given bottles of formula against my wishes. It was as if the nurses felt they knew better for my baby than I did. I undermined my confidence as a new mother.


Even after we left the hospital, we continued to have difficulties. I had a mild infection on my incision. My son had a bacterial infection that caused blisters. It began on his scalp where they had inserted the monitor, but soon covered the area under his fat little arms. He also developed a severe case of thrush that made breastfeeding a complete nightmare. It was over two months before my sore and cracked nipples healed. I can clearly remember sitting down on the couch with my son to breastfeed and crying in pain.


Eventually I moved on emotionally or so I thought. I realised the truth when over eighteen months later I found out that I was pregnant once again. All of my old fears and feelings re-surfaced. I sat and cried for hours; not because pregnancy was unplanned or the additional strain that another child would place upon our finances or troubled relationship, but because I would have to endure another caesarean. I began to look for alternatives and eventually found a midwife that would consider a homebirth. Our first pre-natal visit lasted almost three hours and was more about debriefing from the trauma of my first birth than my physical condition.


But even the successful VBAC homebirth of my 7 lb. 14 oz. daughter did not lessen my anger at what I thought was an unnecessary intervention. In fact, a casual comment by my midwife that my pelvis was more than adequate caused my anger to intensify. In a classic Post Traumatic Shock Disorder experience, that comment re-ignited all the feelings that I had experienced right after the surgery. I tried to channel my anger in a constructive manner. I became a crusader for natural birth; taking a lay midwifery course. I tried to find an attorney to sue my doctor, but the statute of limitations had expired.


For five years after my caesarean, I often wondered about the what-if's. What if I had stuck with my plan to deliver at the other hospital with its less intrusive procedures? What if I had walked about or laboured standing up? What if I was not continuously monitored? Every time I got caught up in those what if's, I would become angrier or depressed. Few people have the true benefit of hindsight, but I was blessed with a chance to once and for all settle those what if's.


Those questions were answered once and for all with my second homebirth. This son was considerably larger than my daughter weighing 8 lbs. 15 oz, but still smaller than his older brother. I had a very short and intense labour of less than two hours. I had what many would consider an ideal natural birth. I laboured upright and walked about through out early labour. I pushed in a position comfortable to me. I had a wonderful support person in my midwife and her assistant. I was comfortable in my home, but my son became stuck in the birth canal. My experienced midwife said that his shoulder dystocia was one of the worst that she had dealt with in her decades of practice. She felt that prayer was the only thing that saved my son. After birth, his breathing was depressed and his one minute APGAR was barely 4. All of the what-if's that had haunted me for five years were gone. It is sad to say but it was that experience that allowed me to release the anger at my doctor that I had been carrying for almost five years.


It was this experience that made me realise for the first time that cephlo-pelvic disproportion was not some made up condition used by doctors to pressure their patients into unnecessary caesarean sections. I suppose that the permanent ridge on the top of Jeffrey's head where he was wedged against my pelvic arch should have indicated this sooner, but it is often easier to blame others than to accept our own liability. I began to accept that my caesarean had been the result of overeating in pregnancy, which resulted in a baby too large for my pelvis.


My emotional healing was completed over a decade later with the birth of my youngest son. I had planned a midwife assisted hospital birth. I was confident that I would have no difficulty with another VBAC. I stayed home during early labour and went to the hospital only once labour was well established the contractions close together and so intense that I was having difficulty managing them. I was sure based upon my earlier labours that the baby's birth was imminent, but when checked I was only 2 cm dilated.


The baby was posterior and my intense back labour was not effectively dilating my cervix. I tried labour upright in the shower. I tried walking. I tried lying on my side. Nothing worked. The pain was more intense than any of my previous labours. I knew that if I choose to have an epidural I would significantly increase my chances of having another c-section, but the pain was so intense that I made the decision to have it anyway. Several hours and several interventions later, I did indeed have another caesarean. This time though rather than anger and resentment, I owned my decision. I was confident that I had done the best I could for me and my baby.


The other part that was healing for me was that in the fifteen years since my first c-section so much had changed that many of the things I had most hated about the experience just did not apply. When my son was born, they brought him to me to touch before placing him in the warming bed. My partner, his mother and Jeffrey were able to touch and talk to the baby while the surgeons sewed me up. Then I was taken to the recovery and reunited with my son; less than an hour after his birth. The nurses helped me to nurse him immediately. Best of all, the hospital encouraged rooming in for all babies. My partner stayed with us even during the night and the only time my baby was separated from us was for about 15 minutes each day for weighing.


It was as if I had come full circle. Despite the fact that the procedures were the same, I was a different person. I had matured. I had learned my rights and responsibilities. When I was faced with an unfamiliar situation, I knew the benefits and risks of all my choices and I easily accepted the consequences of those choices. I was empowered by the same procedure that had once left me so emotionally scared that I suffered from Post Traumatic Stress Disorder. Some of it was because I had changed and some of it was the result of changes within the medical system.


As a mother who has experienced three caesarean births, I at times become very frustrated with the natural birth movement. Yes, the natural birth of my daughter and despite its difficulties even my second son were wonderful. They in some ways healed my inner woman after the first traumatic caesarean, but neither of them were perfect either. I wrote and carefully discussed elaborate birth plans with my midwives and neither birth followed the plans.


Births almost never follow plans. I spent five years angry and depressed for absolutely no reason. How many other women have experienced this pain which inflicts a scar much deeper than a caesarean birth? I have often wondered what would have happened if that shoulder dystocia had not occurred. Would I have continued to be angry at my doctor and depressed? Would my PTSD ever completely healed? Of course, I will never know the answers to those questions. And I feel deeply for the tens of thousands of women who never have the clear answers that fate provided me.


But my experience has led me to begin childbirth education classes and doula services specifically for these special women and babies for whom caesarean birth is chosen. I believe that with compassionate support and care the breastfeeding rates among these families can be dramatically increased and more importantly we can help them to heal quicker both physically and emotionally. This is my unique vision and mission; my long journey back from birth trauma.

What are the symptoms of PTSD do most damage?

PTSD patients would give to an average of 13 years of life without the disorder--but may not realize that some symptoms have no effect on happiness


In the case of post-traumatic stress disorder symptoms, are not all created equal.


In an effort to better treat PTSD, is a study published in the journal psychiatric services 1 March the first to examine the problems associated with PTSD actually correspond to lower quality of life, as indicated by the patient's willingness to die earlier or risk their life-threatening treatment to relieve symptoms.


PTSD is more expensive than any other anxiety disorder. As many as 300,000 veterans returning from Iraq and Afghanistan currently have PTSD, for their concern with costs estimated at $ 4 to $ 6.2 billion over the next two years.


"To our knowledge, this is the first study actual patients with PTSD questions about their health preferences," said lead author Jason Doctor, Associate Professor of pharmaceutical economics and policy at the USC School of pharmacy. "These findings identify targets for treatment that can improve the quality of life among people with PTSD."


The researchers found that, of four significant symptoms associated with PTSD, not all associated with a patient's immediate quality of life were, even though those who treat PTSD significant overall declines in mental health care.


Painful memories of a traumatic event and the avoidance of certain activities and thoughts-understood both often as dysfunctional behavior – had little correlation to a patient reported feeling of well-being, according to the study.


Symptoms associated with increased arousal – such as sleep problems, irritability and vigilance – were associated with lower quality of life in patients of PTSD. Anxiety and depression were also associated with lower quality of life.


"We suspect that while avoiding an important aspect of PTSD, the impact on the quality of life may be limited because it is a coping strategy. Avoidance can be a dysfunction which, in the short term, subjective well-being, "explained doctor – the faculty examination of Schaeffer Center for health policy and Economics at USC.


The researchers found that, on average, a patient with PTSD was ready to give to live by the symptoms of the disorder unequivocally 13.6 years of his or her life.


On average, those with PTSD were prepared to accept sponsorship from a treatment with up to a 13 percent chance of immediate death to full exemption of PTSD symptoms.


Lori Zoellner of the University of Washington, Seattle and Norah widened from Case Western Reserve University were co-authors of the study.


The researchers interviewed 184 persons seeking treatment for PTSD treatment on two sites in different regions of the United States. The research was funded by the National Institute of Mental Health.

Anxiety treatment-natural approach to curing the concern and uneasiness

Anxiety Introduction
Anxiety is a natural human feature of behavior and is an inherent part of our species. The word "anxiety" is derived from the word "anxious", which came from a meaning of being tormented. Anxiety in itself is then known to be a type of self-torment which is brought up as a defensive response to a given situation.
Anxiety is so common that almost everyone experiences it in some form every single day. This anxiety is not a problem, it is only when anxiety begins to creep into places where it actually has no real justification that it becomes a problem and is termed to be a 'disorder'.


Anxiety Disorders
There are a range of different types of anxiety disorders which may be treated differently. The most common anxiety disorders include:

Common Anxiety Causes

Genetic Predisposition - Some types of anxiety disorders such as Generalized Anxiety Disorder (GAD) have been found to run in families and are thought to perhaps be inherited through a gene which makes people more prone to become anxious. Stress - High levels of stress or seemingly endless stressful situations have been proven as a cause of anxiety disorders. Disorders such as Post Traumatic Stress Disorder (PTSD) are specifically found to have been caused by stressful and traumatizing events. Sudden life changes - Death of loved ones and other sudden, shocking changes in everyday life can play a large role in the development of an anxiety disorder.


There may also be a range of internal and external factors which are involved in the development of each specific case of anxiety disorders. It is important to talk about the things you think might be causing your anxiety problems with someone who will listen and understand where you are coming from. This can help you to resolve your own feelings and to better understand why you are going through a tough time.


Common Symptoms of Anxiety
Symptoms of anxiety disorders may range vastly depending on the type of anxiety. Some very common anxiety symptoms include:

Cognitive therapy

If the person says to themselves, "I am boring" it is dis-empowering and may worsen the condition. If the person can begin to change their beliefs and surrounding thought patterns, it becomes much easier to change for the good.


Cognitive therapy includes rational 'self-talk', reality testing, cognitive challenging, attention training and cognitive restructuring. This includes monitoring your self-talk, challenging unhelpful fears and beliefs, and testing out the reality of negative thoughts.


Correct breathing techniques
Something as simple as learning how to change your patterns of breathing can be of great benefit to reducing the effects of your anxiety. Hyperventilation is a key physiological problem which results in the body becoming over-saturated with oxygen and thus panic-stricken. Learning to breathe through the diaphragm instead of the chest is greatly helpful here, simply allowing the stomach to expand with each breath.


You can make sure you are breathing correctly by placing one hand on your lower abdomen and the other on your chest. Correct breathing means your abdomen moves, rather than your chest. It also helps to slow your breathing while feeling anxious. You can also try to hold your breath for a few seconds. This helps to boost carbon dioxide levels in the blood and counteracts the effects of hyperventilation.


Behavior therapy
A major component of behavior therapy is exposure. Exposure here simply means, removing all obstacles which are standing between you and your fear and confronting it at its most basic level. This may help to re-assess and redefine fears which might then cause a breakthrough.
The steps of exposure therapy may include:


* Rank your fears in order, from most to least threatening.


* Decide to start on one of your least threatening fears.


* Perform an imaginary role-play of confronting the fears and analyze what it is exactly that is causing fear.


* Slowly increase your tolerance to confronting your fear by taking small steps towards being more and more afraid or uncomfortable.


* Resist the urge to leave. Use relaxation, breathing techniques and coping statements to manage your anxiety.


* Reflect on your result from the situation and realize that nothing bad happened.


* Repeat the exposure as often as you can to build confidence that you can cope.


* Perform the same cycle of exposure with other fears when you are ready.


Dietary adjustments
Changes made to regular dietary consumption can also make a big difference to the severity of your anxiety problems. Reducing the intake of caffeine is one of the most basic starting points, remembering that caffeine can be found in moderate to high doses in coffee, tea, chocolate, green tea and energy drinks. Taking more health-promoting, calming substitutes for these foods can make a huge difference in itself. Other things you can do are simple things such as eating more fruits and vegetables and eating foods which have not been highly processed. Try to avoid white breads, margarine, deep fried foods and chocolate bars.


Exercise
Exercise helps to promote the production of happy-hormone endorphins which make you feel good. On top of this, exercise will help to destroy stress hormones which are responsible for the "fight-or-flight" reactions our bodies have to stressful situations (also a key cause of anxiety). Aim to do a varied exercise program each week at least 3 days per week and keep it up!


Learn to be more assertive
Letting people know how you feel about certain things may help to avoid loads of unnecessary stress which would otherwise be placed on you! You need to become more assertive in your communication with other people which means communicating your opinions, needs and wants more directly. This will help to build your own self esteem and also just avoids lowering your self esteem by not speaking up!


Building self-esteem
Creating an environment of people and things that make you feel good about yourself will help to build your self-worth and feelings of confidence will arise as a result. With anxiety, many people typically find that low self esteem is inherent. This low self esteem can then create a compounding effect which can enhance feelings such as:


* Isolation


* Shame


* Guilt


* Depressed mood


* Difficulties in functioning at school, work or in social situations.


Structured problem solving
Learning how to actively seek out solutions to your problems really can be half the battle in dealing with your anxiety. Many people who suffer from anxiety remain so passive and un-motivated to solve their problems that they become drowned in worry and do nothing about their problems.


Medication
Drugs have been shown to be effective in treating anxiety while they are being taken. It is vitally important, however, to view medications as a short-term solution to the problem only. It has been clearly proven in medical publications that behavior therapies and regular counseling sessions when coupled with natural anxiety treatments are far more effective in defeating anxiety in a long-term way.

Reduce the risk of Post signs of stress

For the past several years, teams of psychologists and counselors have been deployed to areas where natural or violent incidents have occurred in order to intervene with survivors of these traumatic events. Yet, much of this has been done without any real understanding of the process of surviving trauma, and with all of their good intentions, may have actually increased their likelihood of the development of PTSD.


Historically, the interventions that have been tried with survivors have often resulted in increasing the delayed anxiety and nightmare symptomology, over the years, doctors have routinely dispensed barbiturates or benzodiazepines to assist people getting through the initial days immediately following the traumatic incidents. This has, unfortunately resulted in increasing the likelihood of developing a delayed stress reaction, psychologists armed with this knowledge then swooped down on populations surviving major trauma in an effort to help them process their feelings. However, this too failed to produce the results which they worked diligently, often with no remuneration, to accomplish.


While at this time there are no guaranteed steps to use to avoid development of the debilitating disorder, there are certain measures that can be taken to reduce the risk, or even decrease the symptomology. What we know now is that getting a trauma survivor to talk in depth about the event without time to process and assimilate the experience actually increases the likelihood of development of PTSD symptoms. This information is crucial for the support systems of these survivors to be armed with in their efforts to provide support. Now armed with this information, helpers can intervene with responses that are actually able to assist with the resolution of the trauma. It is important, in working with survivors immediately after a traumatic event, to help them re-establish a sense of control. Simple questioning which do not require in-depth responses are helpful. Ask "What name would you like for me to call you?", "could I get you a glass of water?" or "Where would you like to sit?"


DO NOT ask them to immediately begin describing the events as this can actually increase the likelihood of developing PTSD. DO tell them that their reactions are a natural response to a traumatic event. They should be reassured that while the traumatic experience creates a sense of chaos in their lives, they are not actually "crazy". If the person is going to be involved in a criminal investigation, then informing them of what will be expected of them, and providing them with the details of the course of events which will follow will assist them in feeling more in control.


Further, education about PTSD can help to alleviate fear of "going crazy" if they understand that their responses are normal, and frequently experienced by survivors of trauma. They need to understand that the reactions they have, while intense, are NOT evidence of them "going crazy", but instead indicate that they are responding normally to an abnormal event. They also need to be reassured that their is no shame in seeking psychological help from time to time, especially in periods of high stress. Treatment can actually quickly reduce traumatic stress symptoms and prevent decay of the survivors functioning.


Certain self care measures may decrease the likelihood of development of PTSD. Shortly after a traumatic even has occurred, it is important for the survivor to take the time to restore a sense of control and meaning to their lives. For the first days immediately after the event, they should be encouraged to NOT discuss the event details, but instead to make contact with family and friends in some kind of normal setting which will restore a sense that life can go on even in the face of such a horrible experience. This will also allow them to re-establish a sense of being connected to other people in an important way. Spending non-stressful time, in the company of people who care about the survivor will provide a sense of purpose and meaning to life which at this point may be in serious question for the survivor.


Societal response to a traumatic event is frequently to make efforts to get the survivor to medicate the feelings of shock and despair which are the initial responses to the event, it is critical to NOT medicate with drugs, (either prescribed or not) or with alcohol. Diving back full force into a heavy work or personal responsibility schedule will also increase the possibility of developing PTSD later on.


The stages of trauma resolution are similar to the stages of grief. When trauma can be resolved in a healthy way shortly after the traumatic incident, the possibility of developing PTSD symptoms is minimized. Each of the stages has a predictable and negotiable chain of responses. Navigating these successfully will frequently ease the emotional burden of the traumatic events.


Because shock is the first response to any type of trauma, the numbing may feel confusing, even shameful at first. Yet it, too, is a normal response to an abnormal occurrence. This numbing may also cause the survivor to fall into the belief that they are "handling things" and not be in need to the preventative or self care measures recommended. Education about the stages of recovery from a traumatic event will often help the survivor recognize what is happening and not fall into the trap of believing that they can "handle it" because their feelings are not problematic at first. While it is true that some people do seem to have an innate ability to manage trauma without developing PTSD symptoms, there is no guarantee that it will be them. Paradoxically, it may be that the fact that the survivor has survived other traumas without developing PTSD symptoms actually increases the likelihood of the development of PTSD this time. it is as if people have a limited number of traumas they can handle in a lifespan, and when something happens to a person who has prior trauma as a child, with no ill effects, an adult trauma is actually more likely to trigger a PTSD type response. This means that if a person has had childhood trauma, they are more likely to develop PTSD as an adult if they are again exposed to a traumatic event.


Then, as feelings begin to be freed, the pain is often overwhelming, uncontrollable crying, or shaking, or fits of rage are not uncommon to survivors working through the next stages of the process. Anger tends to be right on the surface just underneath the shock and denial, sometimes this response takes the form of lashing out at those closest to the survivor, and damaging those relationships which are most needed for support. Finding healthy outlets for the anger is crucial to moving into the next phase of the healing. When the trauma has a violent, or criminal component, the rage is often incapacitating, without an outlet: the anger can take over the person's life. Anger outlets such as hitting a punching bag, tearing up newspaper, hitting the bed with a tennis racket, are just a few of the creative solutions survivors have developed to release these overwhelming feelings of rage.


Tears and intense sadness will often follow quickly on the heels of the anger outbursts, once the anger is released, the sadness, despair, and pain of what occurred becomes prominent. The crying may become debilitating at times, but there IS an end to the pain. When the horror of what occurred can be processed fully, the ability reconnect with the things that really matter to the survivor's previous sense of self can be restored. The grief can be overwhelming, and the pain can feel as if it will never end. But, the pain will end. and it will not make person "crazy". The pain itself can be worked through a layer at time. Frequently this processing of the pain takes large amounts of time to work through. That doesn't invalidate the point that it will eventually come to a completion and no longer take over the person's life.
